Documentation Guidelines for Medical, Mobility,
Or Sensory Disabilities
The letter or report should come from the appropriate
licensed professional, such as the treating physician, orthopedist,
physical therapist, ophthalmologist, audiologist, speech pathologist, as
appropriate. Please see the
General Documentation Requirements for other
important information about the source of documentation, as well as the
report itself.
Documentation must be current, preferably no more than
two years old. However, depending on the nature of the disability, more
recent documentation may be requested. In certain circumstances, with
permanent, stable conditions, older documentation may also be
acceptable. The purpose of the documentation is to substantiate the need
for services based on the student's current functioning.
In order to determine eligibility for services, the
University requires specific information about the student's impairment
and how this impairment creates a
substantial limitation to a
major life activity. Toward that end, we request the following
information:
- Clearly stated clinical diagnosis, ruling out alternative
explanations/diagnoses
- Description of this student’s current
symptoms, with approximate date of onset and last contact with the
student, if relevant
- Identification of the major life activities
affected by this disorder such as walking, learning, seeing, hearing,
sleeping, etc. (The severity of the condition and substantial impact caused by
the disability is necessary in order to determine eligibility.)
- Current treatments/medications, including side
effects or other impact of the treatments/medications
- Recommendations for accommodations to equalize
this student's educational opportunities at the postsecondary level.
Accommodations must be logically supported by the documentation; therefore, if
the connection is not obvious, inclusion of the rationale is encouraged. The
intensity of duration of the accommodations, if temporary or highly variable,
should be addressed.
- Any other information relevant to the
student's individual needs.
